2026 marks the 6th year of DSAMn’s Family Connector Program- time flies when we’re connecting with families and working hard to support the Down syndrome community! Not sure what the Family Connector Program is or need a refresher on how Family Connectors can help? Here’s snapshot of what we do and who we are:

What is the Family Connector Program?

DSAMn’s Family Connector Program is a go-to resource when you have questions or concerns about navigating programs and care for your loved one with Down syndrome. Family Connectors provide support, assistance and quality referrals to individuals with Down syndrome and their caregivers through all stages of life- diagnosis through adulthood. The Family Connector program started in 2020 and has since served over 1,500 support requests.

What do Family Connectors do?

Family Connectors work 1:1 with families to help navigate complex systems such as disability benefit programs, healthcare, education and more. Family Connectors explain programs and processes, provide resource connections and walk alongside families through many questions and topics. Here are some common requests that come through to our program:

  • How do I apply for medical assistance? Does my child qualify for SSI?

  • Can I get a PCA for my child? What’s a waiver?

  • We’re having issues at school and we don’t know what to do.

  • We’re struggling with behavior and looking for support.

  • I’m looking for activities for my child/teen/adult.

  • Is there a Down syndrome clinic?

  • We need financial help.

  • My child is turning 18 soon. What should we be thinking about for the future?

Family Connectors also help run DSAMn’s Emergency Financial Assistance Program, as well as see families in person at Down Syndrome clinic appointments.

Who is the Family Connector team?

DSAMn has a diverse team of Family Connectors to support the diverse population and needs of the Down syndrome community in Minnesota and surrounding areas. Our team members bring specific background and professional experiences to the Family Connector Program to provide specialty focused support to families. Our specialties include:

  • New parent support

  • Transition to adulthood

  • Adulthood and aging

  • Bilingual (Spanish)

  • Black parent support

  • Rural family support

  • Financial assistance programs

The Family Connector Program uses a team approach. Family Connectors collaborate together weekly to share ideas and expertise to best serve families. Family Connectors also participate in on-going professional development from a variety of sources such as Disability Hub and the Minnesota Department of Human Services to stay up-to-date with processes, current happenings that impact our community and resources available.

What is it like working with a Family Connector?

The Family Connector Program follows an intentional case management model to serve DSAMn families. The first step to connecting with our program is to fill out a Support Request Form. An intake is then completed and matched with a team member based on request needs. You will work with the same Family Connector throughout your request to ensure personalized support and efficiency. Your Family Connector will listen to your questions and concerns, and offer suggestions for next steps to take. They will share ideas and connect you to resources. They will do research, make phone calls, and help you communicate with other organizations. Your Family Connector will check in with you every couple of weeks to make sure you are getting the support that is needed. When your request is complete, the door to the Family Connector Program remains open. We encourage families to reach back out to us at any time when more help is needed.
